To apply for ODSP (Ontario Disability Support Program) diapers in Ontario, you can follow these steps:

1. Reach out to your physician or visit your local Ontario Disability Support Office to obtain a Mandatory Special Necessities Benefit Form (MSN).

2. After approval by your caseworker, Flowercity Pharmacy will furnish your caseworker with a quotation for your monthly essential supplies.

3. Remember to provide your caseworker with the following details: Name, Phone/Fax number, and member ID number.

4. Once the quotation is endorsed by the caseworker, they will transmit the information to us via fax at (905) 450 1684.

5. Upon completion of the quotation and all necessary paperwork, a Our Pharmacy team member will get in touch with you to provide further instructions.
