To transfer your prescription to us, please fill out the form below with your prescription details.

Submit your prescription transfer request to Flowercity Pharmacy.

Please allow up to 2 hours for us to process your prescription transfer request.

We will notify you once your prescription is ready for pickup.

    Your Current Pharmacy

    Enter the name and cross-streets or address of your current pharmacy. You can find their info the top of the prescription label on your medication.

    We'll contact them directly on your behalf and transfer all your prescriptions to FlowerCity Pharmacy, hassle-free.
